to you. At your convenience, we ask that you fill out the
enclosed forms and bring the completed forms on your first
visit.

your comprehensive exam and treatment recommendation, our business
staff will be happy to assist you in understanding your financial
responsibilities. We do file all major insurance claims electronically
on your behalf. Financial arrangements can be made on larger
amounts and we do accept Master Card, Visa, and Discover for
your convenience. We also offer Care Credit, providing affordable rates as well as long-term financing for larger cases. Applications can be made online. |
